Coronary microvascular dysfunction is associated with reduced cardiac performance in low flow low gradient aortic stenosis

چکیده
Abstract Background Little is known about coronary microvascular function of patients with low-flow low-gradient aortic stenosis (LFLGAS). We hypothesized that LFLGAS associated more severe dysfunction (CMD) compared normal-flow high-gradient (NFHGAS) and CMD related to reduced cardiac efficiency. Purpose To perform a prospective invasive assessment in undergoing TAVI compare it NFHGAS. Moreover, we aimed assess the possible acute impact on interactions between performance at speckle tracking echocardiography (STE). Methods Invasive thermodilution-based was systematically performed 41 consecutive isolated AS angiographic unobstructed arteries TAVI. The index microcirculatory resistance (IMR), resistive reserve ratio (RRR) flow (CFR) were derived before after Advanced echocardiographic imaging, including STE, function. Results IMR significantly higher NFHGAS (24.1 [14.6–39.1] vs 12.8 [8.6–19.2] p=0.002). Similarly, RRR lower (1.4 [1.1–2.1] 2.6 [1.5–3.3] p=0.020). No significant differences observed CFR two groups. High low stroke volume (rho=−0.427, p=0.005), output (rho=−0.517, p=0.001), peak atrial longitudinal strain (PALS) (rho=−0.610, p≤0.001) presence fibrillation (54.6% 21.1%, p=0.036). Conversely, only modestly mean pressure valve gradient (rho=−0.304, p=0.054). Notably, group (rho=0.632, p=0.003) but not (rho=−0.222, p=0.333). high AVA (rho=−0.50, p=0.025) (rho=0.157, p=0.497). Cardiac resynchronization therapy for low-flow, low-gradient aortic stenosis.
Low-flow, low-gradient aortic stenosis is a heterogeneous entity that encompasses truly severe aortic stenosis as well as mild-to-moderate aortic stenosis in which aortic valve orifice area is severely reduced primarily due to left ventricular (LV) contractile dysfunction. Under such circumstances the capacity of the LV to generate stroke-work is severely compromised. In this case report, we de...
متن کاملParadoxical Low-Flow, Low-Gradient Aortic Stenosis
Low-flow, low-gradient aortic stenosis (AS) is a highly challenging condition in terms of diagnosis and therapeutic management. The transvalvular pressure gradient is inversely related to the square of aortic valve area (AVA) and directly related to the square of flow. Hence, a patient with severe AS may nonetheless present with a low gradient if his or her left ventricular (LV) output is reduc...

Correspondence to: Dr John Chambers, Cardiothoracic Centre, St Thomas Hospital, London SE1 7EH, UK; jboydchambers@ aol.com _________________________ A ortic stenosis is thought to have a long, asymptomatic latent phase during which the risk of sudden death is low. In fact symptoms can be revealed by treadmill exercise in a large proportion of apparently asymptomatic patients. Patients may limit...
متن کاملParadoxical low-flow, low-gradient severe aortic stenosis despite preserved ejection fraction is associated with higher afterload and reduced survival.
BACKGROUND Recent studies and current clinical observations suggest that some patients with severe aortic stenosis on the basis of aortic valve area may paradoxically have a relatively low gradient despite the presence of a preserved left ventricular (LV) ejection fraction. The objective of the present study was to document the prevalence, potential mechanisms, and clinical relevance of this ph...
متن کاملPredictors of Outcomes in Low-Flow, Low-Gradient Aortic Stenosis
Background—Patients with low-flow, low-gradient aortic stenosis have a poor prognosis with conservative therapy but a high operative mortality if treated surgically. Recently, we proposed a new index of aortic stenosis severity derived from dobutamine stress echocardiography, the projected aortic valve area at a normal transvalvular flow rate, as superior to other conventional indices to differ...
